What you’re getting yourself into: the evidence on high protein and healthy kidneys, why your blood test will look worse than your kidneys actually are, the one test that gets around it, and exactly what to ask your GP.

Twenty eight studies. 1,358 people. Pooled, compared, picked over. No adverse effect of higher protein intake on glomerular filtration rate in healthy adults, which is the single number the entire question turns on (Devries et al., Journal of Nutrition, 2018). ESTABLISHED.
That is the science settled, and it settled a while ago.
The science was never going to be your problem.
Your problem is the printout.
Because carnivore will make your kidney blood test read worse while your kidneys are doing absolutely nothing wrong, and if you do not understand why before the surgery rings you, you will spend a fortnight awake at three in the morning reading about transplant waiting lists and quietly working out whether your brother would be a match.
The kidneys are fine. The dinner was the problem, along with the shoulders.
One boundary before we go further, and this is not small print. Everything in this article is about healthy kidneys. If you have chronic kidney disease, a single kidney, or diabetes that has already touched your kidneys, your nephrologist outranks this website and every other website in existence. This piece will make that conversation sharper. It will not replace it.
Does high protein damage healthy kidneys?
This fear has been studied half to death, which is convenient, because it means nobody here has to speculate.
The Devries meta-analysis compared higher protein intakes, 1.5 g per kilo and upwards, some of them far upwards, against normal and lower intakes. The conclusion, in the authors’ own words, was that higher protein intakes “do not adversely influence kidney function on GFR in healthy adults”. ESTABLISHED.
What higher protein does do is make healthy kidneys work.
In a controlled crossover trial, young men eating 2.4 g per kilo for a week saw their filtration rate climb from around 125 to 141 ml per minute (Frank et al., American Journal of Clinical Nutrition, 2009).
An organ responded to demand.
Your legs do the same thing under a loaded bar, and nobody has ever examined a hypertrophied quadricep and diagnosed leg failure.
The clinical nutrition literature calls this a normal physiological adaptation to nitrogen load, and notes that claims of high protein straining the kidney “remain speculative” (Martin et al., Nutrition and Metabolism, 2005). ESTABLISHED for the adaptation. The strain story has roughly the evidentiary standing of the one about your face staying that way if the wind changes.
For completeness, because this site does not hide the inconvenient rows: in that same Frank trial, urinary albumin also nudged upwards during the high-protein week. Short trial, healthy men, and nobody has since shown it means harm. But it happened, so it goes on the page. OBSERVED.

Why your blood test will look worse anyway
The standard kidney check is serum creatinine, dropped into a formula that estimates your filtration rate. The result is called eGFR, and that small “e” is doing an enormous amount of unacknowledged work.
Estimated. A calculation, not a measurement. A suit bought off the peg for a man of average build who does not exist.
Creatinine is a waste product of creatine. Creatine lives in muscle. Including, and this is the bit that undoes everything, the muscle you eat. Cooking converts it into creatinine directly, you absorb it over dinner, and it turns up in your bloodstream looking identical to the creatinine your own body made.
Somebody measured this properly, and the result should be laminated and pinned to the wall of every surgery in the country.
Eighty people ate a standardised cooked meat meal and had their bloods tracked across the following four hours. Serum creatinine jumped. eGFR fell. At every stage of kidney function tested.
Six of the sixteen patients who began the day classified as CKD stage 3a were reclassified as stage 3b within two hours of the meal (Nair et al., Diabetes Care, 2014).
Same kidneys. Different lunch.
Nothing about those six filters changed in a hundred and twenty minutes. They ate, and an equation decided they had got worse. The effect vanished after twelve hours of fasting. ESTABLISHED.
Creatinine also tracks the muscle you carry around permanently, and that half does not wash off overnight. In 170 healthy people, lean mass was significantly related to serum creatinine, independent of diet and activity (Baxmann et al., Clinical Journal of the American Society of Nephrology, 2008).
The formula assumes you have average muscle.
It assumes you eat an average diet.
You have more muscle than average and you have eaten meat within twelve hours of every blood draw you have ever given in your adult life. You fail both assumptions, in the same direction, on the same morning, in the same vial.
The creatinine reads high. The formula reads that as a labouring filter. The printout says your kidneys are worse than they are.
Nothing malfunctioned except the arithmetic’s opinion of you.
The test that gets around it
There is a second way to estimate filtration that does not care what you had for dinner or what you can pull off the floor.
Cystatin C is a protein produced by almost every cell in the body at a steady rate, and in the Baxmann study, muscle mass moved creatinine and left cystatin C entirely alone. ESTABLISHED as the mechanism-independent alternative.
The guidance has wandered about, and you want to know where it stands before you ask. NICE once recommended cystatin C testing to confirm borderline CKD diagnoses, then withdrew that recommendation, judging the accuracy evidence insufficient and noting that few UK centres ran it routinely anyway (NICE, 2014, withdrawn 2021). The international kidney guideline body went the other way. KDIGO recommends combined creatinine and cystatin C estimates where they are available, and specifically where creatinine is unreliable and the answer matters (KDIGO, 2024).

DISPUTED at the guideline level, which is worth knowing in advance, because your GP may well quote NICE at you and they will be quoting it correctly.
In practice, the NHS will usually not run it off the back of one borderline reading. UK private labs do offer it, though you will not find it hiding on the mass-market home-test panels. Whether it is worth your money is precisely the sort of question the end of this article exists to arm you for.
What about urea?
Urea is the other number that climbs, and it climbs for the least mysterious reason in medicine.
Urea is how the body packages nitrogen for disposal. Protein is nitrogen. Production is linearly related to protein intake (Martin et al., 2005). In the Frank trial, a week of high protein raised it exactly on schedule, in healthy men, alongside kidneys that were filtering more rather than failing.
ESTABLISHED, and benign, in healthy kidneys.
The boundary bears repeating in every section it touches. A raised urea in someone with existing kidney disease is not something you get to wave away with this paragraph. Diet explains a high urea in a healthy person. It does not explain one away in a sick one. That is a doctor’s call, every single time.
What about kidney stones?
Two different stones. Two different answers.
Most stones are calcium oxalate, and dietary oxalate comes almost entirely from plants. Spinach. Nuts. Seeds. Tea. The usual suspects, wearing halos, sitting in the salad. On carnivore your dietary oxalate intake falls to somewhere around nothing. Uric acid stones are the smaller category, and they belong to the transition period, when urate can run high for a while. That story sits with the gout article, and the practical advice is the same unglamorous advice it always is. Drink water like an adult, particularly in the first few months.

The one to respect is ACR.
Everything else on that table has an innocent carnivore explanation waiting for it. Albumin in your urine does not. If that number is raised and stays raised across repeat testing, that is not a formula being unfair to your ribeye habit.
That is the one honest witness in the room. Your job at that point is to be a boringly obedient patient.
Who this article is not for
Once more, without decoration.
Existing chronic kidney disease. A single kidney. Diabetes with kidney involvement. Blood in your urine. Swelling in your legs. Any actual symptom, as opposed to a number you have taken a dislike to.
All of that is GP and nephrologist territory. High-protein diets in kidney disease are a genuinely different scientific question, the evidence above does not cover it, and anyone who tells you it does is doing you harm while sounding confident about it.

The questions to take to your GP
Walk in with these and you will have a completely different conversation to the one where you get told off by a stranger for eating steak.
- “Can I fast for twelve hours before the blood draw, with no meat the evening before?” This one question removes the entire meal effect (Nair et al., 2014). Book morning bloods, skip the steak dinner, stop poisoning your own test.
- “Given my muscle mass and my diet, would you expect my creatinine to run above the reference range?” You are not arguing with the result. You are asking whether the formula’s assumptions describe you. Doctors answer that question willingly. They do not enjoy the other one.
- “What is my ACR?” If it is normal, say less and feel smug quietly.
- “Is my eGFR trend stable?” One reading is a snapshot of your dinner. A stable trend across years is information.
- “Would a cystatin C based estimate be worth doing in my case?” Expect a no on the NHS. Now you know why, and you know what a private test would and would not settle.
A doctor who watches you sit down with those five questions treats you as a participant rather than a liability. That is the entire trick, and it takes about ninety seconds.
Carnivore Kidney Questions
Does a carnivore diet damage your kidneys?
Not if they are healthy. A meta-analysis of 28 studies covering 1,358 people found no adverse effect of higher protein intake on kidney function in healthy adults.
Why is my creatinine high on carnivore?
Cooked meat contains creatinine and raises your blood level for up to twelve hours, and carrying more muscle raises it permanently. Both make the standard test read worse with no kidney damage involved. Fast for twelve hours before your next draw.
Is a low eGFR on carnivore always a false alarm?
No. It is often explained by meat and muscle, but the way to find out is a fasted retest, your ACR, and the trend over time, with your GP. Never diagnose yourself with either disease or innocence off a single number.
What blood test shows real kidney damage on carnivore?
ACR, the albumin to creatinine ratio, is the marker with no dietary excuse available to it. A persistently raised ACR needs a doctor regardless of how good you feel.
Should I get a cystatin C test?
It is the estimate that ignores muscle and meat, recommended in international guidance where creatinine misleads (KDIGO, 2024), though NICE withdrew its recommendation (NICE, 2021) and the NHS rarely runs it. Available privately in the UK.
Can I do carnivore with chronic kidney disease?
That is a question for your nephrologist, not a website. The evidence for healthy kidneys does not transfer to kidney disease.
Book the bloods for a Monday morning.
Fast the twelve hours. Skip the steak the night before, once, on purpose, for a reason that has nothing to do with guilt.
Take the five questions in on the back of a receipt.
The creatinine will still read high, because you have the shoulders of someone who takes Tuesdays seriously, and that was never the thing anybody needed to worry about.
Then go home and have the steak.
